Item Double Chamber Right Ventricle in a Patient With Supracristal Ventricular Septal Defect and Prolapsing Right Coronary Cusp: Role of Intraoperative Transesophageal Echocardiography(JOURNAL OF THE AMERICAN SOCIETY OF ECHOCARDIOGRAPHY, 2009) Gadhinglajkar, S; Sreedhar, R; Krishnamanohar, SR; Jayant, A; Bhoomkar, NSA supracristal ventricular septal defect (VSD), an outlet VSD situated in subaortic and subpulmonic regions, has a strong association with prolapse of the aortic valve cusp. The authors report the case of a patient operated for a supracristal VSD with prolapse of the right coronary cusp. The VSD was found to be in the subaortic position on preoperative transthoracic echocardiography, which failed to detect the presence of a double-chamber right ventricle. Intraoperative transesophageal echocardiography correctly recognized the supracristal nature of the VSD and identified the double-chamber right ventricle, subsequently altering the course of surgery. (J Am Soc Echocardiogr 2009;22:754.e3-754.e5.)Item Star in the heart(HEART, 2004)In a 50 year old woman, transthoracic echocardiography showed a left atrial mass. Transoesophageal echocardiography delineated its attachment. Additionally, cystic spaces were seen arranged concentrically in the shape of a star in the centre of the tumour. Surgical excision followed. Histopathological examination confirmed myxoma with areas of haemorrhage and necrosis. This case highlights the acoustic property of myxomas in a rare and beautiful manner and emphasises the superior transoesophageal imaging of myxomas.Item Teddy bear in the heart(INTERNATIONAL JOURNAL OF CARDIOVASCULAR IMAGING, 2004)In a patient with native aortic valve endocarditis, transoesphageal echocardiography yielded a teddy bear appearance which is not reported so far. A perivalvular abscess ( right ear), the superior vena cava in cross section ( left ear) and the dilated ( post-stenotic) aortic root ( face) made up the teddy bear. This was not a cuddlesome toy but an ominous sign.
