A case of thoracic spinal cord compression caused by intradural extramedullary fungal granulomatous lesion associated with acute lymphatic leukaemia is presented. The specific diagnosis on imaging was difficult and required specialized histopathological studies. A high index of clinical suspicion of fungal infection, surgical decompression of spinal cord, and tissue biopsy confirmation are particular features of our case.
